How to place a request for disclosure and other matters related to your personal information:
		1.	Visit our website and access the "Contact Us(on Japanese)" page. You will be asked to fill out a request form. Upon receiving your request, we will send you the necessary documents by regular mail.
		2.	For submitting a request for notification of the purposes for our use of personal information or a request for disclosure of your personal information, you are asked to pay a 1,000 yen handling fee plus 1,220 yen for actual expenses including postage. To place a request for other items than those above, you are asked to pay 1,220 yen for actual expenses including postage.
		3.	The above-mentioned payment will be accepted in the form of a postal money order (including fixed postal money orders).
		4.	You will be asked to submit to us a copy of your ID (including driver's license) so that we can verify that the request has been made by the customer himself/herself or by someone authorized by him/her to do so.
